The Conflict of Interest and Conflict of Commitment Disclosure Report for Support Staff can be found on the HRS website. Support Staff who are required to complete a disclosure report should fill in the form and submit it to their assigned supervisor within their faculty or department.
Administration of the annual disclosure reporting process and retention of associated records is the responsibility of the respective faculties and departments. Supervisors are responsible for soliciting and monitoring the annual submission of disclosure reports.
At this time, there are no changes to the disclosure reporting process for Academic Staff.
